PLEASE NOTE:
Booking a trial day at Meaningful Connections allows participants to have a short term experience of the program and allow them the opportunity to see if it would be a good fit for them and their care partners. 
We ask that care partners of participants fill out this intake assessment form from the Department of Social Development prior to booking a trial day, as the process can take some time.
While we absolutely welcome trial day bookings without completing the application, being approved by the department will allow full access to the program at Meaningful Connections beyond the trial day.
